Home Health Physical Therapist/PT Job - Rockville, MD

Our client is a hospital system in the Rockville, MD area. We are searching for a full time physical therapist for a direct hire, home health position. Most of the caseload is adult and geriatric patients. The physical therapist will evaluates, plans and implements physical therapy treatments to patients in the home care setting as prescribed by a physician. Communicates with team members, customers, managers, etc. to coordinate plan of care and provide optimal care. 2. Administers physical therapy interventions and treatments according to agency's accepted policies and procedures. 3. Demonstrates appropriate therapy knowledge and plans patient care with appropriate patient goals. 4. Completes documentation requirements 5. Administers and supervises physical therapy treatments to patients with a wide range of health problems under the direct orders of a physician/podiatrist. 6. Assumes responsibility for personal and professional development.Required Knowledge, Skills, Abilities, and Other Characteristics:- Graduate of an approved school of physical therapy- Maryland Licensure- CPR certification and preferred continuing education, including workshops which are related to homecare.- Minimum of one year experience, preferred prior home care experience- Speaks, reads, writes English
