Exciting Opportunity in the Tempe, AZ area: Home Health Physical Therapist – PT
Position Summary:
We are seeking a licensed Physical Therapist to provide skilled home health services to patients throughout the Tempe and surrounding East Valley communities. The Physical Therapist will evaluate, develop, and implement individualized treatment plans designed to improve patient mobility, strength, balance, safety, and overall functional independence in the home setting.
Essential Responsibilities:
- Perform comprehensive physical therapy evaluations in patients' homes.
- Develop individualized treatment plans based on physician orders and patient needs.
- Provide therapeutic exercises, gait training, balance training, transfer training, and patient education.
- Assess home environments and recommend safety modifications and assistive devices.
- Collaborate with physicians, nurses, occupational therapists, speech therapists, and caregivers.
- Complete all required documentation accurately and timely in accordance with agency, state, and federal guidelines.
- Monitor patient progress and modify treatment plans as appropriate.
- Participate in interdisciplinary care conferences.
Qualifications:
- Graduate of an accredited Physical Therapy program.
- Current Arizona Physical Therapist license in good standing.
- Current CPR certification.
- Minimum one year of physical therapy experience preferred.
- Home health experience preferred but not required.
- Valid driver's license, reliable transportation, and current auto insurance.
- Strong communication and documentation skills.

Coverage Area:
Tempe, Mesa, Chandler, Gilbert, Scottsdale, and surrounding communities.
Schedule:
Monday–Friday, daytime hours. Weekend rotation may be required.

Director of Clinical Services
